If you have a provider that you would like to seek treatment from and they are not contracted with Valenz Access, or you are not finding a particular provider specialty represented within your area, you can complete a Provider Nomination Form online and submit it directly to Valenz. The Valenz contracting team will then determine if the provider is eligible to be added to the network.
