Providers are responsible for verifying Member eligibility prior to providing medical services. Eligibility can be obtained by contacting the Member Services telephone number indicated on the Member's identification card. Claim status must also be checked by contacting the Member Services telephone number indicated on the Member’s ID card. Valenz does not determine benefits eligibility or availability for Members and does not exercise any responsibility with respect to policy, payment, practices or procedures.
